Analytics

For each visitor to reach the site, we expressively collect the following non-personally identifiable information, including but not limited to browser type, version and language, operating system, pages viewed while browsing the Site, page access times and referring website address. This collected information is used solely internally for the purpose of gauging visitor traffic, trends and delivering personalised content to you while you are at this Site.

Information provided is stored separately to website data on Amazon Web Services (AWS) infrastructure located in Australia, via a third party provider, Snapforms, chosen for their security and reliability. We do not collect your credit card details, bank details, Medicare or Private Health Insurance details online.

If you complete a form on this website, we will use this information to contact you and if required, generate a medical record or a contact record. These records are maintained within our Clinical Practice Software. You may request for a contact record to be deleted. Medical records are required to be maintained by law for a minimum of seven years from last entry for an adult and for children until they have reached the age of 25 years.

 

Website administrators and website developers cannot see information provided in our forms.

What rights you have over your data

You have the right to access your personal information, subject to exceptions allowed by law, and can request to receive an exported file of the personal data we hold about you, including any data you have provided to us. You are required to put your request in writing for security reasons. You can also request that we erase any personal data we hold about you. This does not include any data we are obliged to keep for administrative, legal, or security purposes. SWAE Holdings Pty Ltd reserves the right to charge a fee for searching for, and providing access to, your information on a per request basis.
